By submitting your login username and password, you agree to be bound by all terms of this Member Agreement.

The interpretations provided by LabInsight are for informational purposes only. As a
LabInsight participant, you assume full responsibility and risk for the appropriate application and use of this health information. No information provided by the LabInsight service is intended to diagnose, treat, or follow-up on any medical disease or condition, nor is it intended to replace an actual "hands-on" consultation with your existing qualified healthcare provider.

REMEMBER, THE LabInsight SERVICE IS NOT A SUBSTITUTE FOR A CONSULTATION WITH YOUR PHYSICIAN!

Always check with your health care provider regarding any questions or concerns that you have regarding your health. The LabInsight service does not engage in doctor-patient relationships with its participants. LabInsight strives to improve the existing doctor-patient relationships through empowering the member with relevant medical information. LabInsight, assumes no liability whatsoever for the information provided, or for any diagnosis or treatment made in reliance thereon.

Participation in the LabInsight service is strictly voluntary and done solely at your own risk.

The information provided through the LabInsight application is the property of LabInsight and is protected by applicable copyright and patent laws. As a member, you agree to not reproduce, retransmit, distribute, disseminate, sell, publish, broadcast, or circulate the information provided to you. This includes, but is not limited to, translation into foreign languages, storage on CD-ROM's, and any other use. You may display, reformat, and print any information for your own personal, non-   Commercial purposes. 

LabInsight will use reasonable and HIPAA compliant measures to protect the personally identifiable information you provide through the LabInsight application from loss, misuse, and unauthorized access, disclosure, alteration, and destruction.

We will not give, sell, rent, or loan any of your personally identifiable information which you provide through this service to any third party, unless legally required to do so or unless you specifically direct us to release this information to your health care provider.

Your personalized results will be available to you after you've completed the blood draw and other necessary actions to produce lab results.  A third party agent of LabInsight hosts the registration page and the entire LabInsight service. The LabInsight software analyzes your lab measures and stores this data on the servers owned by the hosting company. The hosting company has agreed to return all personally identifiable information from the LabInsight service.

Please be aware that it is possible that technical staff at the hosting company will see your answers while helping LabInsight solve technical problems. LabInsight does reserve the right to use, sell, and/or share with other third parties, non-personal, summary, or aggregate data for business analysis, market analysis, communications management, research, or planning purposes. Such information will also be used to help improve the quality of the LabInsight service.

Due to the unpredictable hazards of electronic transmission and distribution, there may be inaccuracies, omissions, and typographical errors in the provided information which are unknown to LabInsight.  Despite daily error review,  LabInsight can not warrant the completeness or accuracy of any of the information. We are committed to providing accurate information; however, at times, as with all medical information, the information may incorporate expert opinions and experienced medical judgments which LabInsight has not developed and for which LabInsight can not be responsible.

Neither LabInsight nor any of its parents, subsidiaries, affiliates, agents, licensees, or their respective officers, directors, employees, agents, contractors and assigns shall be liable to you or anyone else for any loss or injury caused in part or whole by situations beyond its control in procuring, compiling, interpreting, providing, and storing of your medical information or resulting from  your reliance or the reliance of your healthcare provider by your or its reliance upon the information provided by the LabInsight service.

You are responsible for safeguarding your unique password. All data modifications to your records with respect to your password will be your responsibility and presumed to have been executed with  your consent. This Member Agreement is personal to you and your rights or obligations may not be transferred to another person. LabInsight reserves the right to modify, amend, restate, and/or terminate the Member Agreement at any time and post these changes to the membership information area. It remains your responsibility, as a member, to periodically review the Member Agreement and to contact the LabInsight within seven business days if you do not accept the amended Member Agreement. Your failure to notify LabInsight that you do not accept any changes in the Member Agreement made by LabInsight means that you accept the terms of the new Member Agreement and that the terms of the new Member Agreement will apply to you. If any provision of this Membership Agreement is deemed invalid, then the remaining provisions will continue in full force and effect. This Membership Agreement, all intellectual property issues, and your rights and obligations shall be governed by the laws of the United States of America and the State of Tennessee.

By clicking on the "I Agree" button displayed under the login, you (i) represent that you are at least 18 years of age; (ii) acknowledge that it is solely your responsibility to follow-up with the appropriate medical provider for any medical services you think you need as a result of participating in the LabInsight application; (iii) release and hold harmless LabInsight, its parents, subsidiaries, affiliates, agents, and licensees and their respective officers, directors, employees, agents, contractors and assigns of any responsibility and/or liability pertaining to the use, administration and/or nonuse of your responses to the LabInsight service; (iv) agree that any changes made to this Member Agreement in the future will apply to you and your relationship with LabInsight, (v) acknowledge that it is your responsibility to review the Member Agreement from time to time to determine if any changes have been made; and (vi) agree that you will be legally bound by and abide by this authorization as if you had signed it.

PLEASE DO NOT PARTICIPATE IN THIS LabInsight SERVICE IF YOU DO NOT AGREE TO THE TERMS AND CONDITIONS OF THIS AUTHORIZATION!
