- Access Code is used only once for your initial registration.
During the registration process, you will create a username and password
and provide contact and health information.
- After registering, use your username and password to log in. (You
will no longer need the Access Code for your personal login.)

- Each individual (including family members) must be registered
separately to access any service (LabInsight/Health Risk
Assessments, Appointments, etc.)
- For security, if you go more than 25 minutes without touching your
keyboard or mouse, your confidential session will timeout and all entered
data will be deleted. You will need to login again. Thank you.
